Position: Certified Practical Nurse (LPN)

Key Responsibilities:

• Manage the admissions, discharges, and transfers of residents.
• Coordinate medication orders with pharmacies and healthcare providers.
• Provide medication administration as directed.
• Comply with infection control standards, emergency policies, and other center protocols.
• Work alongside the interdisciplinary team to promote optimal health outcomes for residents.
• Identify and communicate any significant changes in residents’ conditions to the relevant personnel as per center policy.

Qualifications:

• Must have a valid Licensed Practical Nurse (LPN) license issued by the state.
• CPR certification is required.
• Preferred candidates will have at least two years of nursing experience.
