Position: Licensed Practical Nurse (LPN)

Key Responsibilities:

• Manage admissions, discharges, and transfers of residents.
• Coordinate medication requests with pharmacies and physicians.
• Administer medications as prescribed.
• Adhere to infection control protocols, emergency procedures, and other facility policies.
• Work collaboratively with the interdisciplinary team to achieve optimal patient outcomes.
• Monitor and report any significant changes in residents' conditions to the appropriate personnel according to center policy.

Qualifications:

• Must hold a valid Licensed Practical Nurse (LPN) license in the state.
• CPR certification is required.
• Preference given to candidates with two years of prior nursing experience.
