Clinical Practitioner LPN

Responsibilities for the Licensed Practical Nurse (LPN):

• Managing the admission, discharge, and transfer of residents
• Coordinating medication requests with physicians and pharmacies
• Administering prescribed medications
• Adhering to infection control practices, emergency procedures, and facility policies
• Collaborating with the interdisciplinary care team to ensure the best care outcomes
• Notifying appropriate staff about any changes in residents' conditions, in line with facility protocols

LPN Qualifications:

• State-issued LPN license
• CPR certification required
• A minimum of two years of nursing experience is preferred
