Addiction Care Nurse Practitioner

We are on the lookout for a compassionate Addiction Care Nurse Practitioner with a Suboxone waiver to join our growing team. You will provide care for adults dealing with substance use and mental health disorders, performing comprehensive evaluations and guiding them through detox and rehabilitation treatment plans. Your role will be crucial in determining the right care approach for each patient.

Primary Duties:

• Evaluate patients by gathering medical history and conducting health assessments based on presenting symptoms.
• Diagnose and identify patient needs by analyzing clinical data and history.
• Perform waived testing such as urine, blood, and glucose monitoring to assist in diagnosis.
• Collaborate with a physician to create an individualized, comprehensive treatment plan.
• Administer and manage the agreed-upon treatment plan, including tests, medications, and referrals for additional care.
• Monitor the effectiveness of treatments and adjust care plans as necessary to meet mental health goals.
• Provide patients with educational resources and information to help them make informed decisions about their health care and treatment.
• Foster a secure, supportive environment, advocating for patients at every level.
• Work closely with colleagues to deliver well-rounded, comprehensive patient care.
• Maintain thorough, accurate, and confidential patient medical records.

Qualifications:

• Master's Degree in Nursing.
• Licensed Nurse Practitioner in New York with a current NP certification.
• Ability to write prescriptions, with DEA licensure and compliance with NY Drug and Alcohol Program standards.
• Current BLS certification.
• At least five years of experience in a related field.
• ANCC certification and credentialing.
